Personalized Health Program
You may voluntarily elect to participate in the Personalized Health Program offered on our Site, a complete program that delivers a personalized health risk assessment (HRA), genetic testing, and customized nutritional supplements tailored to your personal health and nutrition needs. The HRA collects Personally Identifiable Information, including personal health information concerning your nutrition, lifestyle, and health history. The Site automatically processes your HRA information through an algorithm specially designed to analyze your nutritional needs, develop nutritional supplement recommendations, and perform safety checks for toxicity and allergic reactions. HRA information is securely stored in your separate HRA profile and accessible only by logging into the Site with your Username and password. Note that if you share your account with others (spouse, family member, business partner) with whom you are registered under the same Username and password can also access your HRA information on the Site. To protect your personal health information we encourage you not to share your account and to safeguard the password you use to access our Site as described in the “Protecting Your Information” section, below. Except as disclosed in this section, we do not share your HRA information with any non-affiliated third party without your permission.
This refers to information that does not, by itself, identify you as a specific individual. Such information would include the Uniform Resource Locator ("URL") of the Web site that referred you to our Site, your Internet Protocol ("IP") address (a number automatically assigned to your computer whenever you surf the Web), your operating system and browser type, and any search terms that you enter on our Site.
